B. Instructions
Please fill out this form as complete as possible, including a letter of recommendation from your academic advisor and/or instructor who should also be a member of 2026 IOHSK. Any missing and/or incomplete item will disqualify your application for further review and inclusion. For grant submissions, applicants are required to review the grant submission policy carefully.
Both scholarships and research grants require 2026 IOHSK registration with paid membership. Applications submitted without a valid 12-digit IOHSK membership number will not be considered. All scholarships and research grants are awarded based on the IOHSK membership merit system.

D. Eligibility
(a) 2026 IOHSK Registered Undergraduate/Graduate Student for Scholarships.
(b) 2026 IOHSK Registered Faculty and Graduate Student Joint Research Grants.
(c) 2026 IOHSK Registered Faculty Grants with IOHSK Fellow(s).
2026 Membership Application & Membership Payment:
Required to apply for the scholarship or grant.

Individuals who have previously received this award (scholarship or grant) are also eligible to apply; however, priority will be given to applicants who have not received a scholarship or grant in the past. The total number of awards will be determined based on the number of qualified applications and the 2026 fiscal budget. In addition, IOHSK has implemented a membership merit system to support registered and paid members in 2025 (after April 1, 2025) to 2026 (January 1 to current). E. Letter of Recommendation
Please note that each registered and paid faculty member is allowed to write a maximum of two (2) letters of recommendation per year, which can be for scholarships, grants, or a combination of both (e.g., 2 for scholarships, 2 for grants, or 1 for each scholarship and grant).
Please ensure that your faculty advisor/supervisor is a registered and paid "faculty" member of IOHSK in 2025-2026 to be eligible to make a recommendation for scholarships and grants.
F. Grant Submission Policy

2026 IOHSK Registered Faculty and Graduate Student Joint Grants.
For grant submissions, the Principal Investigator (PI) must be a faculty member. A letter of recommendation from a colleague, supervisor, or Chair/Dean is required. Each submission may include up to two additional faculty members and one to two graduate students. The total number of investigators must not exceed five.
Grant submissions must be made by the Principal Investigator (PI) and must include the following information for all investigators: full names, 12-digit IOHSK membership numbers, academic status (faculty, masterโs, or doctoral), email addresses, and affiliated university.
Grant recipients are required to register, attend, and present their final research at the 2027 IOHSK International Conference (Location as of now is Tokyo, Japan; late June 2027). Virtual registration and participation will be permitted if in-person attendance is not possible. Grant funds for attending the 2027 IOHSK International Conference may be applied toward travel, accommodations, and registration fees. Failure to meet the presentation requirement will result in the full return of the grant award by April 30, 2027.
Research projects must include physical activity as one of the study variables and must be data-based. The Dr. Thomas McKenzie Research Grants were established under this policy in 2021.

Each submission must include at least one FIOHSK (Fellow of IOHSK) and may include up to three additional faculty members. The total number of investigators must not exceed five.
Grant submissions must be made by the Principal Investigator (PI) and must include the following information for all investigators: full names, 12-digit IOHSK membership numbers, academic status (full-time and tenure track faculty only), email addresses, and affiliated university.
