1. Five (5) colored photocopies of your School ID. Each copy must have three (3) live signatures using a blue ballpen

2. Three (3) copies of the Certification. You may print copies in advance OR request copies from Sir Kyle. Completely fill out all required details and sign each copy.

3. Validated School ID for verification.

📌 FOR GRANTEES WHO CANNOT CLAIM PERSONALLY
Authorized claimants must bring:

1. Five (5) colored photocopies of the validated grantee’s School ID. Each copy must have three (3) live signatures using a blue ballpen

2. Signed Authorization Letter from the grantee (blue ballpen)

3. Five (5) colored photocopies of any Government-issued ID of the claimant. Each copy must have three (3) live signatures using a blue ballpen

📌 FOR DECEASED GRANTEES

The claimant must present:

1. Annex 6: Claim Form (to be secured from the Admission and Scholarship Office)

2. Death Certificate of the grantee

3. Five (5) colored photocopies of any Government-issued ID of the claimant. Each copy must have three (3) live signatures using a blue ballpen

The House of Representatives under the leadership of Speaker Faustino “Bojie” Dy III today approved on third and final reading a measure that aims to ensure that prepaid and postpaid internet consumers get the full value of their data subscription.

House Bill No. 87 or the proposed “Roll-Over Internet Data Act” seeks to mandate Internet Service Providers (ISPs) to implement a Roll-Over Data Allocation Scheme for their subscribers. With this, consumers are assured that their unused data allotment shall not be forfeited but carried over to succeeding months and can be converted at yearend into rebates which they may also avail and use.

Likewise approved on third reading is HB No. 5939 seeking to discontinue the mandatory implementation of spiral progression approach in the K to 12 curriculum across all subjects. The measure also gives the Department of Education the free hand to determine the most relevant and effective curriculum approach for K to 12 to ensure mastery of knowledge and skills after each level.

The Chamber also passed a bill of local significance, HB No. 5663 which aims to rename the University of Science and Technology of Southern Philippines (USTSP) into the University of Science and Technology of the Philippines (USTP).
