𝗡𝗼𝘁𝗲: In compliance with CTU Memorandum No. PJ06-311, s. 2025, the surrender of old student IDs for those who already graduated is MANDATORY at the SAO Office. Lost or misplaced IDs must be supported with an Affidavit of Loss.

2️⃣ Valid ID of the Alumni
3️⃣ Valid ID of the Representative
