𝗡𝗼𝘁𝗲: In compliance with CTU Memorandum No. PJ06-311, s. 2025, the surrender of old student IDs for those who already graduated is MANDATORY at the SAO Office. Lost or misplaced IDs must be supported with an Affidavit of Loss.

Please ensure your records are updated and keep the Endorsement Slip issued upon submission, as this will be required for your Alumni ID application.
