๐ฆ๐๐ฒ๐ฝ ๐ฏ: Prepare the original hard copies of the requirements listed hereunder. Scan and save them in portable document format (pdf).
1. Notice of Admission to the Program (NOAP)
2. Form 138-A (Report Card for SHS graduates) / Honorable Dismissal and Transcript of Records with "for evaluation/reference copy remark" (for transferees only)
3. Signed USeP Student Information Sheet
4. Signed Data Privacy and Non-Disclosure Agreement (DPNDA) Form
5. Filled out Student ID Information Form
6. Two (2) pcs. Passport-sized I.D. picture with collar in white background and with nametag
7. Two (2) pcs. Unlabeled thick long brown envelopes for documents (10 inches x 15 inches)
8. Signed Promissory Note Form (to be submitted if items nos. 9-12 below are not yet available)
9. Medical certificate issued by the University Clinic
10. Dental certificate issued by the University Clinic
11. Good Moral Certificate
12. PSA/NSO issued Birth Certificate (original and photocopy)
13. PSA/NSO issued Marriage Certificate (for married female students only)
14. National Commission of Indigenous People Certificate (for IPs)- if applicable
15. PWD I.D. (If applicant is a PWD)
16. Solo Parent I.D. (If applicant is a solo parent)
17. OWWA OFW e-card/Overseas Employment Certificate (OEC) (If the parent/s of the applicant is/are OFW/s)
18. Mailing stamps worth Php 50.00
Note: Students must wait for an announcement from the University Clinic as to how requirements 9 & 10 are to be secured.
๐ฆ๐๐ฒ๐ฝ ๐ฐ: Submit in person the ORIGINAL HARD COPIES of the requirements mentioned in Step 3 to the Office of the University Registrar/Campus Registrar. In case some of the requirements are not yet available, the students must sign a promissory note and submit it along with requirements 1-7.
*Requirements 1-7 must be submitted during the PREENROLLMENT period. **Requirements 9-18 must be submitted during the ENROLLMENT period.
๐ฆ๐๐ฒ๐ฝ ๐ฑ: Register in the Pre-Enrollment System using this link: http://srrs.usep.edu.ph/preenrollment/ and upload the scanned requirements mentioned in Step 3. The link will be activated only during the pre-enrollment schedule. To register, the student must use his/her Student Application Number and Application Password assigned during the admission application.
๐ฆ๐๐ฒ๐ฝ ๐ฒ: The student shall receive his/her USeP student I.D. number through email. He/she must monitor the USeP official website and page for specific instructions on ENROLLMENT, which is scheduled on June 15, 2026 โ July 23, 2026.
