First, kindly accomplish the Course Evaluation Form for second semester and summer, AY 2023-24. This is your chance to give us your feedback from the 2nd semester and summer courses you took for the continuous improvement of our delivery of instruction. Also, this is a REQUIRED step prior pre-enlistment and enrollment.

After evaluating the courses you took this past 2nd semester and summer, kindly accomplish the PRE-ENLISTMENT FORM for the FIRST SEMESTER, AY 2024-25.
