Attention, incoming freshmen!

📝 𝐄𝐱𝐭𝐞𝐧𝐬𝐢𝐨𝐧 𝐨𝐟 𝐄𝐧𝐫𝐨𝐥𝐥𝐦𝐞𝐧𝐭: Enrollment is extended until August 7, 2026. Please make sure to complete your enrollment requirements and procedures on or before this date.

Bring Original and 2 copies of the following:

• Good Moral Certificate
• Form 138 or Grade 12 Report Card
• Form 137
• PSA Birth Certificate
• Photocopy of Marriage Certificate (if Married)
• DEPED Certificate and Certificate of Rating (for ALS and PEPT Passer)
• Transcript of Record and Honorable Dismissal (for transferee)
• 3pcs of 1x1 Picture (formal attire with white background)
• 2 Long Blue Folder
• Long transparent Envelop
