To ensure a smooth and hassle-free enrollment, bring the requirements and follow the steps!

✔️Original Copy of Card/Form 138
✔️Photocopy of PSA Birth Certificate
✔️Certificate of Honors (Rank 1 and Rank 2)
✔️1 copy 2x2 picture (for medical form)

✔️Step 2: Document Verification
Head to the Faculty Room (Window 1).
Submit your printed Pre-Enrollment Form and required documents to the teacher-in-charge.
At this stage, you will also select your preferred payment scheme.

✔️Step 3: Subject Encoding & Approval
Once verified, proceed to the High School Registrar’s Office.
Your subjects will be encoded, and your Pre-Registration Form will be officially approved.

✔️Step 4: Payment Validation
Proceed to Window 5 to have your payment details verified before heading to the cashier.

✔️Step 5: Settlement & Official Registration
Wait for your name to be called at Window 3 or 4 to settle your fees.
📌Crucial: After payment, return to the Faculty Room to claim your OFFICIAL REGISTRATION FORM.

✔️Step 6: Medical Scheduling
Finally, visit the Health and Wellness Center to schedule your Medical Assessment. Once your schedule is set, your enrollment is complete!
