• CSU College Admission Test (CAT) Result
• Form 138 / Grade 12 SHS Card
• Certificate of Good Moral Character
• Original or Authenticated Copy of PSA Birth Certificate
• Two (2) copies of 2x2 ID Picture (taken within the last six months)

📝 𝐄𝐍𝐑𝐎𝐋𝐋𝐌𝐄𝐍𝐓 𝐏𝐑𝐎𝐂𝐄𝐃𝐔𝐑𝐄

1. Download and print your CAT result at portal.csu.edu.ph
2. Proceed to your assigned college/department for an interview and evaluation of requirements
3. Submit your complete documents to the Registrar’s Campus Admission Office - CSU Sanchez Mira

IMPORTANT:

Please make sure you have two (2) photocopies of all your documents ready. Keep your original copies arranged in the order listed, as these will be submitted to the Registrar’s Office.

📌 Don’t forget to place everything in a long brown envelope, as required by the University Admissions Office.
