All students are advised to carefully review and comply with the following enrollment requirements based on their classification.

• Certificate of Good Moral Character
• PSA Birth Certificate (Original), 2 Photocopies (1 for Student Affairs, 1 for Department)
• Medical Certificate
• Medical certificate must have official seal or official receipt
• Must be in Bond Paper size (Short / A4)
• 2 pcs. 1x1 ID Picture
• 2 pcs. Long Brown Envelope
• 2 Long Blue Plastic Folder

Good day.

To ensure an orderly and efficient enrollment process, all students are advised to strictly observe the official enrollment flow. Please prepare all required documents and complete each step in the prescribed sequence.

𝐄𝐍𝐑𝐎𝐋𝐋𝐌𝐄𝐍𝐓 𝐅𝐋𝐎𝐖

1. 𝐑𝐨𝐨𝐦 𝐌 𝟏𝟎𝟐 – Issuance of Enrollment Form / Submission of Enrollment Requirements
Secure the Enrollment Form and submit the required documents for admission. This step is intended for all students to verify and complete their enrollment requirements.

2. 𝐑𝐨𝐨𝐦 𝐌 𝟏𝟎𝟑 – Signing of School Nurse / Submission of Medical Certificate
Submit the Medical Certificate and obtain health clearance from the School Nurse. This ensures compliance with the school’s health and safety requirements.

3. 𝐑𝐨𝐨𝐦 𝐌 𝟏𝟎𝟒 – Signing of SSG
Obtain the signature or clearance from the Supreme Student Government (SSG). This confirms student acknowledgment of school organizations and activities.

4. 𝐂𝐎𝐄𝐝 𝐅𝐚𝐜𝐮𝐥𝐭𝐲 𝐎𝐟𝐟𝐢𝐜𝐞 – Information Encoding / Updating, Printing of Grade Slip, Evaluation, and Tabulation
Encode or update student information, print grade slips, and evaluate academic records. Faculty members will assess subjects and verify curriculum requirements for enrollment.

5. 𝐂𝐚𝐬𝐡𝐢𝐞𝐫 beside EDP Office / Trinidad Municipal Hall, Treasurer’s Office – Registration Fee Payment (for NON-UNIFAST students ONLY)
Pay the required registration fees for students not covered under the Free Higher Education program. Official receipts must be secured after payment.

6. 𝐑𝐨𝐨𝐦 𝐌 𝟏𝟎𝟗 – Booklet Releasing
Claim the official enrollment booklet and other necessary enrollment documents. These serve as references for enrollment and school procedures.

7. 𝐒𝐩𝐞𝐞𝐜𝐡 𝐋𝐚𝐛𝐨𝐫𝐚𝐭𝐨𝐫𝐲 – Processing of School ID (For New & Transferees)
Process the School ID through photo capture and recording of student information. This step is required for new and transferee students.

8. 𝐑𝐞𝐠𝐢𝐬𝐭𝐫𝐚𝐫’𝐬 𝐎𝐟𝐟𝐢𝐜𝐞 – Submission of Enrollment Form
Submit the completed and signed Enrollment Form for final verification and recording. This officially confirms the student’s enrollment for the semester.
