WHO and UNICEF launch cholera vaccination campaign in northwest Syria amidst earthquake response

Cairo/Amman, 8 March 2023: The World Health Organization (WHO) and the United Nations Children’s Fund (UNICEF), in coordination with health authorities, the Syria Immunization Group (SIG), and the Global Alliance for Vaccines (GAVI), kicked off a cholera vaccination campaign in earthquake-hit areas of northwest Syria.

During the campaign, 1.7 million doses of cholera vaccine will be used to protect Syrians above one year of age, especially those living in the areas most severely impacted by the earthquake and at highest risk of cholera, including Sarmada, Maaret Tamsrin, Dana, and Atmeh districts in Idleb, and in A’zaz district in northern Aleppo.

1400 teams of health workers and community volunteers will implement the ten-day campaign using a house-to-house strategy, as well as reaching displaced people living in camps, markets, and school sites.

"WHO warns against the increase of water-borne diseases for over 2.1 million Syrians living in the northwest of the country, with the risk increasing significantly in the aftermath of the devastating earthquake, especially in overcrowded camps and collective centres" says Dr. Richard Brennan, Regional Emergency Director for WHO's Regional Office for the Eastern Mediterranean. "WHO and partners have to act now to avoid further illness and death. As the cholera vaccines are orally administered, it is crucial to ensure that targeted populations are reached before the beginning of Holy month of Ramadan, as most adults will be fasting during this period".

Situation at a glance

On 7 February 2023, the Ministry of Health and Social Welfare of Equatorial Guinea reported the deaths of a number of individuals with suspected hemorrhagic fever.

On 12 February 2023, one sample was confirmed positive for Marburg virus by real-time polymerase chain reaction (RT-PCR), at the Institute Pasteur in Dakar, Senegal.

Investigations are ongoing to find additional cases. WHO is supporting the response by strengthening contact tracing, case management, infection prevention and control, laboratory, risk communication and community engagement.

WHO assesses the risk posed by the outbreak as high at the national level, moderate at the regional level and low at the global level.

Description of the situation

This is the first Marburg virus disease (MVD) outbreak reported in Equatorial Guinea.

On 7 February 2023, the Ministry of Health and Social Welfare of Equatorial Guinea reported at least eight deaths that occurred between 7 January and 7 February 2023, in two villages located in the district of Nsock Nsomo, eastern province of Kie-Ntem, Río Muni Region. According to the ongoing epidemiological investigation, the cases presented with fever, followed by weakness, vomiting, and blood-stained diarrhoea; two cases also presented with skin lesions and otorrhagia (bleeding from the ear).

On 9 February 2023, eight blood samples were collected from contacts and sent to the Centre Interdisciplinaire de Recherches Médicales de Franceville (CIRMF) in Gabon, where they tested negative for both Ebola and Marburg viruses by real-time polymerase chain reaction (RT-PCR).

The WHO Director-General transmits the report of the fourth meeting of the International Health Regulations (2005) (IHR) Emergency Committee regarding the multi-country outbreak of monkeypox (mpox), held on Thursday, 9 February 2023 from 12:00 to 17:00 CET.

Following a series of consultations with global experts, WHO recommends a new preferred term “mpox” as a synonym for monkeypox. Both names will be used simultaneously for one year while the term “monkeypox” is phased out. This report uses the term “mpox” (for more information see here).

The Emergency Committee acknowledged the progress made in the global response to the multi-country outbreak of mpox and the further decline in the number of reported cases since the last meeting. The Committee observed that a few countries continued to see a sustained incidence of illness; the Committee is also of the view that underreported detection and under-reporting of confirmed cases of illness in other regions is likely. Therefore, the Committee considered various options to sustain attention and resources to control the outbreak and advised maintaining the Public Health Emergency of International Concern (PHEIC), while beginning to consider plans to integrate mpox prevention, preparedness and response within national surveillance and control programmes, including for HIV and other sexually transmissible infections.

The WHO Director-General expresses his gratitude to the Chair, Members, and Advisers for their advice and concurs with this advice that the event continues to constitute a PHEIC for the reasons detailed in the proceeding

Equatorial Guinea confirms first-ever Marburg virus disease outbreak

Brazzaville/Malabo – Equatorial Guinea today confirmed its first-ever outbreak of Marburg virus disease. Preliminary tests carried out following the deaths of at least nine people in the country’s western Kie Ntem Province turned out positive for the viral haemorrhagic fever.

Equatorial Guinean health authorities sent samples to the Institut Pasteur reference laboratory in Senegal with support from World Health Organization (WHO) to determine the cause of the disease after an alert by a district health official on 7 February. Of the eight samples tested at Institut Pasteur, one turned out positive for the virus. So far nine deaths and 16 suspected cases with symptoms including fever, fatigue and blood-stained vomit and diarrhoea have been reported.

Further investigations are ongoing. Advance teams have been deployed in the affected districts to trace contacts, isolate and provide medical care to people showing symptoms of the disease. Efforts are also underway to rapidly mount emergency response, with WHO deploying health emergency experts in epidemiology, case management, infection prevention, laboratory and risk communication to support the national response efforts and secure community collaboration in the outbreak control.

WHO is also facilitating the shipment of laboratory glove tents for sample testing as well as one viral haemorrhagic fever kit that includes personal protective equipment that can be used by 500 health workers.

“Marburg is highly infectious. Thanks to the rapid and decisive action by the Equatorial Guinean authorities in confirming the disease, emergency response can get to full steam quickly so that we save lives and halt the virus as soon as possible,” said Dr Matshidiso Moeti, WHO Regional Director for Africa.

Situation at a glance

Malawi is experiencing a widespread cholera outbreak, with 36 943 cases and 1210 associated deaths reported from all 29 districts since 3 March 2022. This is the deadliest outbreak of cholera in the country’s history.

Cholera has been endemic in Malawi since 1998 with seasonal outbreaks reported during the rainy season (November through May). However, the current outbreak has extended through the dry season, with cases being reported since March 2022.

In light of the ongoing rainy season, wide geographical spread, and a consistently high case fatality rate (CFR) of above 3%, the ongoing cholera outbreak was declared a public health emergency by the Malawi government on 5 December 2022. Currently, the large geographic spread and the high number of reported cases in the country are stretching all capacity to respond to the outbreak, increasing the risk of serious public health impact. The outbreak in Malawi is occurring against a backdrop of a surge in cholera outbreaks globally, which has constrained the availability of vaccines, tests, and treatments.

With a sharp increase of cases seen over the last month, fears are that the outbreak will continue to worsen without strong interventions.

Description of the cases

On 3 March 2022, the Ministry of Public Health of Malawi notified WHO of a cholera outbreak after laboratory confirmation of a cholera case in Machinga district hospital on 2 March 2022.

Between 3 March 2022 through 3 February 2023, a total of 36 943 cases, including 1210 deaths, have been reported from all 29 districts in Malawi (overall case fatality rate (CFR) 3.3%) with active transmission ongoing in 27 of 29 districts. There was a 143% increase in the number of cases in January (17 078) compared to December (7017 cases).
