As discussions around Deskilling, Mis-skilling, and Never-skilling continue, it is equally important to recognize what AI cannot replace in physiotherapy.
AI can support assessments, summarize evidence, monitor progress, and reduce administrative workload. These capabilities can improve efficiency and allow clinicians to spend more time with their patients.
However, physiotherapy is more than information processing or selecting an intervention.
At the core of our profession is the therapeutic allianceโthe collaborative relationship between the physiotherapist and the patient, built on trust, communication, empathy, and shared decision-making. Together with patient-centred care, it remains one of the strongest predictors of engagement, adherence, and meaningful rehabilitation outcomes.
Effective rehabilitation is not simply about choosing the right intervention. It is about delivering the right care, through the right mechanism, at the right time, and for the right person.

The future of physiotherapy is not a choice between AI and clinicians. It is about integrating technology responsibly while preserving the clinical reasoning, empathy, and therapeutic alliance that define our profession.
AI can augment physiotherapy practice, but it cannot replace the human qualities that build clinical competence and meaningful patient care.
