很高興分享:由周建棟教授擔任首席研究員的「RAISE Liver Triage」計劃,獲香港研究資助局(RGC)傑出青年學者計劃(ECS)資助。

香港數據顯示,約73%的糖尿病患者有脂肪肝,18%有肝纖維化。早期肝病未必有明顯病徵,卻可能逐步發展至肝硬化或肝細胞癌。計劃會利用電子健康紀錄,在RAMP-DM、普通科門診及家庭醫學診所識別需要優先接受超聲波、FibroScan®或生物標記物檢查的患者。

研究亦會:
• 追蹤轉介、輪候及診斷延誤;
• 在伙伴醫院驗證風險工具;
• 檢視不同病人群組能否公平獲得評估。

We are pleased to share that RAISE Liver Triage, led by Prof. Jiandong Zhou, has received Hong Kong RGC Early Career Scheme funding.

Liver disease can remain silent in adults with diabetes. In Hong Kong, around 73% have fatty liver and 18% have liver fibrosis, yet timely assessment is often missed.

RAISE will use routine diabetes visits to bring liver-risk assessment closer to patients.

Using existing electronic health records, it will identify who should be prioritised for ultrasound, FibroScan® or biomarker testing, track delays and check whether access is fair.

Simply put: the right patient, the right liver check, at the right time.

We hope to support earlier detection, shorter delays and fairer primary care.

✅ 但目前主要改善嘅係「流程效率」,仲未有明確證據顯示可以直接提升治療效果或診斷準確度

⚠️ 短板與風險 —— 唔好當佢係「全科神醫」

❌ 多數研究仲喺實驗室階段,真係放落社區診所用,仲有好多不確定因素

🤖 AI in Primary Care? HKU Team Breaks Down the Evidence

A new study led by Prof. Edmond Chow from HKU's Department of Family Medicine and Primary Care, in collaboration with Massachusetts General Hospital and MIT, was just published in Communications Medicine. It looks at how large language models (like ChatGPT, Claude) can be used safely in primary care settings.

🔍 What works right now? – Best use is "assisting with the paperwork"

✅ Drafting patient replies, summarising clinical notes, improving doctor-patient communication
✅ Saves time, both doctors and patients report good experiences
✅ But improvements are mainly in workflow efficiency – no solid evidence yet that they directly boost diagnostic accuracy or treatment outcomes

⚠️ Limitations & risks – Don't treat it as a "virtual GP" just yet

❌ Evidence for supporting diagnosis, treatment decisions, or patient outcomes is still very limited
❌ Most studies are still in simulated settings – real-world clinics bring many unknowns
❌ Population health and cost-effectiveness? Still at the "hypothesis" stage – waiting for prospective trials

🔒 Safety first – Four guardrails are essential

🛡️ Privacy, data security, algorithm transparency, and model reliability
🛡️ Current advice: start with high‑volume, low‑risk tasks like documentation and messaging
🛡️ Final decisions and accountability must stay with doctors – AI is only an assistant

💡 Bottom line

AI in primary care has promise, but no rush. Use it for administrative grunt work first, build solid governance and fairness mechanisms, then expand slowly. Doctor in charge, patient at ease – that's the right way forward.

[分享] 歐盟委員會近日更新殘疾人策略,再次展現推動共融的決心。然而,《刺針—基層醫療》提出一個值得深思的問題:這些政策承諾,能否真正改善殘疾人士的健康?
研究顯示,殘疾人士接受癌症篩查的比率較其他人低約20%,醫療需要未獲滿足的機會更高達四倍。這些健康差距並非無可避免,而是反映現行醫療制度和服務設計仍未能充分照顧不同人士的需要。
基層醫療正正是推動改變的關鍵。透過社會處方、跨專業協作及持續照護,家庭醫生能夠連繫醫療與社區資源,為有需要人士提供更全面的支援。然而,若醫療系統資源不足、服務欠缺銜接,再好的理念亦難以落實。要真正建立共融的醫療服務,需要持續投放資源,包括加強醫護人員的殘疾照護培訓、發展無障礙數碼工具,以及改善醫療設施的無障礙環境。
從經濟及公共健康角度而言,推動共融醫療同樣具備充分理據。研究估計,投資於殘疾照護能力培訓,可節省約67億歐元,並有望避免約770萬宗死亡個案。建立更具包容性的基層醫療,不僅惠及殘疾人士,更能促進整個社會的健康公平。

[Sharing] The European Commission’s updated disability strategy signals renewed commitment, but The Lancet Primary Care rightly asks: can this translate into meaningful health improvements?
People with disabilities have 20% lower participation in cancer screening and are four times more likely to have unmet healthcare needs. As family physicians and healthcare leaders, we must recognise that these inequities are not inevitable—they are produced by systems that were not designed inclusively.
Primary care is the critical nexus for change. It bridges healthcare and social needs through approaches such as social prescribing and coordinated care. However, under-resourced and fragmented systems fail those who need continuity of care the most. The solution requires more than policy commitments—it calls for sustained investment in clinician training, accessible digital tools, and inclusive healthcare infrastructure.
The economic case is equally compelling. Investing in disability competence training could save an estimated €6.7 billion and avoid 7.7 million deaths in Europe. Inclusive primary care benefits not only people with disabilities, but the entire community, and is fundamental to achieving health equity.

【港大醫學院研究:控制家庭共同因素後,未發現產前服用撲熱息痛與自閉症或過度活躍症有關】

不少準父母擔心懷孕期間服用撲熱息痛(paracetamol)會增加孩子患自閉症或過度活躍症的風險。由港大醫學院家庭醫學及基層醫療學系助理教授(研究)駱珊領導的研究團隊,分析逾70萬對香港母嬰數據,追蹤長達23年,並採用嚴謹的「兄弟姊妹配對」研究設計,以控制家庭共同的遺傳及環境因素。

研究結果顯示:在控制家庭共同的遺傳及環境因素後,未發現產前服用撲熱息痛與自閉症或過度活躍症有關,而有關結果在不同用藥時間、劑量及使用模式下均保持一致。研究成果已於《JAMA Internal Medicine》發表。

Led by Dr Shan Luo, a new HKUMed study of 708,020 Hong Kong mother–child pairs found that, after accounting for shared familial genetic and environmental factors, prenatal paracetamol use was not associated with autism or ADHD. The findings were consistent across timing, dose, and patterns of use, and have been published in JAMA Internal Medicine.
