Since we r receiving lot of forms filled up incompletely and incorrectly, we r uploading a sampled filled form.

EVERYONE is requested to GET A PRINTOUT OF THE FORM AND THEN FILL IT UP ACCORDINGLY and submit it by 15th of this month.

NB: For 1st Year:
# Course Commencement Date: July 2015
# Completion Date: June 2019
# Year of study I/II/III/IV: II

For 2nd, 3rd Yr, follow the same pattern.

Leave the "Payment Details" blank.
The other details should be filled accordingly.

Fill the form IN "BLOCK LETTERS".

Members are requested to keep the amount (Rs.460/- IN CASH) ready.
