CLUSTer adalah kajian (cohort) di kalangan guru-guru di Malaysia berkaitan dengan pengumpulan faktor-faktor risiko gaya hidup dan tekanan, dan mengikuti perkembangan kesihatan mereka dari semasa ke semasa. Kajian ini akan dijalankan dalam dua fasa aktif (2013/14 dan 2017/18) di mana guru-guru akan menjalani soal selidik soal selidik, pemeriksaan kesihatan dan analisis biomarker. Kemudian peserta a

Kajian ini akan merungkai tentang pertanyaan antara interaksi antara alam sekitar, tekanan dan faktor risiko gaya hidup kepada kesihatan dan kesejahteraan, dan dengan ini seterusnya, dapat memberikan kita hala tuju tentang kepentingan dan tindakan pencegahan pada masa depan. Kelulusan untuk kajian ini telah diberikan oleh Kementerian Pendidikan Malaysia dan Universiti Malaya. Pelepasan etika juga diperolehi daripada Jawatankuasa Etika Perubatan Pusat Perubatan Universiti Malaya (Rujukan No. 950.1). Kajian ini disokong oleh Geran Penyelidikan Berimpak Tinggi Kementerian Pelajaran (MOE HIR No. Guru adalah salah satu kohort kerja terbesar di dunia dan di Malaysia (Kementerian Pendidikan, 2011). Mereka bekerja di sekolah-sekolah yang dilaporkan menjadi persekitaran yang tertekan (Austin, Shah, & Muncer, 2005; Jepson & Forrest, 2006). Menurut kajian oleh Retneswari et al (2011), prevalensi tekanan di kalangan guru sekolah menengah di kawasan bandar di Malaysia adalah 20.2%. Faktor risiko gaya hidup seperti diet yang tidak sihat, tidak aktif fizikal, merokok, penyalahgunaan alkohol, dan tidak mengekalkan berat badan yang sihat dikaitkan dengan penyakit kronik seperti diabetes mellitus, penyakit kardiovaskular dan kanser (Stampfer, Hu, Manson, Rimm, & Willett, 2000; Dana / Institut Amerika untuk Penyelidikan Kanser, 2007; Yusuf et al., 2004). Walau bagaimanapun, faktor-faktor risiko ini biasanya tidak berada secara rawak di seluruh populasi, tetapi berlaku dalam kombinasi antara satu faktor ke satu faktor risiko sesama lain (Poortinga, 2007). Penyelumputan faktor-faktor risiko ini biasanya dikaitkan dengan risiko penyakit yang lebih tinggi daripada yang dijangkakan daripada kesan individu sahaja (Chou, 2008). Kajian terdahulu mendapati bahawa stres dikaitkan dengan risiko penyakit kronik. Stres berkaitan dengan kerja telah dikaitkan dengan risiko sindrom metabolik (Chandola, Brunner, & Marmot, 2006), diabetes jenis II (Heraclides, Chandola, Witte, & Brunner, 2009), penyakit jantung koronari (Eller et al., 2009 (Kivimaki et al., 2006), dan infark miokardia akut (Rosengren et al., 2004). Terdapat banyak kajian yang dijalankan di kalangan guru tetapi kebanyakan kajian ini bukan pada guru sendiri tetapi tentang pelajar mereka. Oleh kerana guru membentuk populasi besar (hampir empat ratus dua belas ribu) di negara kita (Kementerian Pendidikan, 2011) dan penyakit kronik dan tekanan semakin meningkat di Malaysia (Institut Kesihatan Awam, 2008), kini tepat pada masanya untuk mengetahui kesihatan dan kesejahteraan serta faktor risiko gaya hidup mereka dan tahap tekanan. Kajian ini akan memberi penjelasan kepada kita tentang interaksi antara alam sekitar, tekanan dan kluster faktor risiko gaya hidup pada kesihatan dan kesejahteraan, dan memberi kita wawasan tentang kepentingan dan arah tindakan pencegahan masa depan. Di samping itu, guru yang mengambil bahagian akan diberi pemeriksaan kesihatan percuma dan dia akan mengetahui status kesihatannya. Keputusan akan disebarkan kepada mereka dan langkah-langkah pencegahan boleh diambil jika terdapat keperluan. Jika rawatan perubatan diperlukan, rujukan kepada klinik kerajaan atau pengamal am berdekatan akan dibuat untuk rawatan awal. Kelulusan untuk kajian ini telah diberikan oleh Kementerian Pendidikan Malaysia, Kementerian Pelajaran dan Universiti Malaya. Objektif kajian ini adalah untuk membuat tinjauan kepada guru secara meluas di Malaysia berkaitan dengan faktor-faktor risiko gaya hidup dan tekanan, dan akan mengikuti perkembangan guru tersebut dari semasa ke semasa. Kajian ini akan dijalankan dalam dua fasa aktif (2013/14 dan 2017/2018) di mana guru-guru akan menjalani soal selidik soal selidik, pemeriksaan kesihatan dan analisis biomarker. Kemudian peserta akan diikuti dengan panggilan telefon atau kaji selidik mel untuk merekodkan amalan gaya hidup dan aktiviti kesihatan.

| CLUSTer Study : School Engagement Experience |

The CLUSTer study, which began in 2013, is a cohort study that aims to examine the clustering of lifestyle risk factors and work-related stress among teachers in Malaysia, as well as follow up on important health outcomes. The follow-up of this cohort study has resumed since July 2022 and is ongoing in selected schools across Selangor, Kuala Lumpur, Johor, Penang, Terengganu, and Malacca.

We are delighted to engage some selected schools in a health campaign and health screening. On March 15th and 16th, 2023, our CLUSTer study team (Dr. Yoong Lee Yeen, Mr Ng Yit Han, Dr Yap Jun Fai and Dr. Patrick Peng Wee Yao) under supervision of Professor Dr. Moy Foong Ming, has engaged with the selected schools, Sekolah Jenis Kebangsaan (Cina) Subang and Sekolah Jenis Kebangsaan (Tamil) Vivekananda in Petaling Jaya, to raise awareness of teachers' health and wellbeing, conduct blood pressure screening, and administer the CLUSTer RedCAP online questionnaire screening.

The teachers were actively involved in our health talks, health quizzes, X-break sessions, blood pressure measurements, as well as health screening using CLUSTer RedCAP online questionnaire. The teachers' knowledge of non-communicable diseases increased from an average score of 55% in the pre-test to 82% in the post-test after the health talks. We also identified some cases of high blood pressure and provided consultation to the teachers. Through the school engagement programs, we were able to better understand the teachers' working lives and the barriers to healthy living, enabling us to help them incorporate what they learned into their routines.

The school engagement experiences created a bridge for interaction between our team and the teachers, providing meaningful insights for both parties. Metabolic Syndrome among Teachers

Metabolic syndrome was defined using the harmonization criteria as having any three or more of the following risk factors: central obesity, elevated triglyceride, low high-density lipoprotein cholesterol, high blood pressure, and high fasting blood glucose. CLUSTer research findings showed that participants with shorter sleep duration, BMI ≥23.0 kg/m2, and older age had significantly higher odds of metabolic syndrome. The prevalence of metabolic obese, non-obese (MONO), in which the participants who had metabolic syndrome with normal weight was 17.7% among the teacher population.
