1. Photocopy your valid ID.

2. Provide three (3) specimen signatures using blue ink/ballpen on the photocopy.

3. Scan the completed photocopy or take a clear and readable photo of it.

4. Make sure that all information and details are clearly visible.

5. Double-check that you have uploaded the correct and complete ID before submitting.

6. Save the file as a PDF using the following filename format:
LAST NAME, FIRST NAME, MIDDLE INITIAL

Example: `DELA CRUZ, JUAN, P.pdf`
