Notice from the UEP - Office of the Director of Admission:
Good Day, Everyone!
The Office of the Director for Admission of the University of Eastern Philippines Main Campus would like to formally announce that the Applications for Admissions for the Undergraduate Level (Incoming Freshmen, Transfer, Readmission) for Academic Year 2022-2023 are now open.
REQUIREMENTS FOR INCOMING FRESHMEN
1. Expression of Intent for Admission (Form 1)
Note: Form 1 will be pasted at the back of the brown envelope
2. Student Personal Data Sheet (Form 2)
3. Essay (Form 3)

4.1 Authenticated Photocopy of Form 138 – SHS Card (Complete Grades for Grade 11 and First Semester Grades for Grade 12) for graduating SHS students; or
4.2 Authenticated Photocopy of Form 138 – SHS Card (Complete Grades for Grade 11 and Grade 12) for SHS graduates of previous academic years
5.1 Authenticated Photocopy of School Principal Certification as a Candidate for Graduation (for SHS graduating students); or
5.2 Authenticated Photocopy of Good Moral Character Certificate (for SHS graduates of previous academic years)
6. Photocopy of NSO/PSA/Local Birth Certificate (if not available at the time of application, a promissory letter shall be prepared and addressed to the Admissions Committee)
7. 2 copies of latest and colored 2x2 ID pictures with name tag (at least 6 months from the time of application)
8. 1 Long brown envelope (write your name [surname, first name, middle name]at the upper leftmost, back part of the envelope, landscape style)
Note: Documents from #2-8 shall be enclosed in the long brown envelope.

1. Expression of Intent to Transfer (Form 1)
Note: Form 1 will be pasted at the back of the long brown envelope

5. Photocopy of Certificate of Honorable Dismissal
6. Photocopy of Certificate of Good Moral Character
7. Photocopy of Physical Examination (conducted by Government Physician from the applicant’s place of origin)

DEADLINES:
1. 13 MAY 2022: General Admissions
2. 30 APRIL 2022: For Applicants for BS Accountancy and All Engineering Courses
Note: Successful Applicants for BS Accountancy and Engineering Courses will be required to Enroll the Bridging Program this Summer (June-July 2022)
NOTES FOR APPLICANTS FOR READMISSION
1. Applicants who stopped college before Academic Year 2018-2019 (old curriculum) and onwards, shall directly coordinate with the Office of the Director for Admission. Forms are available at the office.
2. Applicants who stopped from Academic Year 2018-2019 and onwards (new curriculum) shall directly coordinate with their Program Coordinator.
3. Readmission to the program is subject to the program's admission and retention policy (i.e. grade evaluation and slot availability).
GENERAL NOTES ON SUBMISSIONS:
1. For Applicants within Northern Samar, submit your documents through the UEP Link. 2. For Applicants outside the province, courier your documents to: The Office of the Director for Admission, UEP, Catarman, Northern Samar.
3. Envelopes with incomplete documentary requirements will not be processed.
NOTES FOR SHIFT APPLICANTS
1. Stay tuned on this page for your schedule.
2. Ensure that your grades are complete so your application can be processed.
3. Admission to the program sought is subject to the new program's admission and retention policy (i.e. grade evaluation and slot availability).
4. Once released by your original degree program you could hardly go back to it as your slot will be given to other shift applicant.
5. Forms will be readily available at the Office for the Director for Admission once the application schedule is posted.
