All applicants who have qualified for the BS Nursing transfer entrance examination are required to submit a copy of their 𝗧𝗿𝗮𝗻𝘀𝗰𝗿𝗶𝗽𝘁 𝗼𝗳 𝗥𝗲𝗰𝗼𝗿𝗱𝘀 (𝗧𝗢𝗥) to the Office of College of Nursing and Allied Health Sciences (CNAHS) 𝘁𝗼𝗱𝗮𝘆, 𝗝𝘂𝗹𝘆 𝟭𝟴 (𝗪𝗲𝗱𝗻𝗲𝘀𝗱𝗮𝘆).

The TOR must be placed inside a brown envelope, with the applicant’s full name clearly written on the upper left corner at the back.
