Attention All BS Hospitality Management Applicants (Main Campus)

Applicants whose status in the student portal is APPROVED are requested to submit the following required documents from July 20–24, 2026 to the Hospitality Management Department Chair at the Faculty Room, College of Business Administration Building.

**Original Copies of the following:
- OLSAT/UEP Entrance Exam Result
- Form 138 - SHS Card (Grade 11 and Grade 12)
- Certificate of Good Moral Character
- Medical Certificate

**Photocopy of:
- PSA Birth Certificate (if not yet available, a promissory letter shall be prepared and addressed to the Registrar

Please prepare two (2) long brown envelopes:
1. One (1) envelope containing the original documents (only photocopy for PSA)
2. One (1) envelope containing the photocopies of all required documents

Using a permanent marker, write your full name on the upper left corner at the back of each envelope (Last Name, First Name, Middle Initial).
