📌 How to Apply:

HM Students: Fill up the Application Form then Submit your Grade Slip (computed) to Mr. Keith Martin Cacanog.

IT Students: Fill up the Application Form then Submit your Grade Slip (computed) to Dr. Rose Ann C. Campita.

Note: No final grades lower than 2.5 in all the subjects. General Average should not be lower than 1.750. Do not include NSTP in the computation.

Location: COT Office

Dates: October 6-29, 2026 (No extension)
