📢 ANNOUNCEMENT TO ALL OLD STUDENTS
(Incoming 2nd Year, 3rd Year, and 4th Year Students)
The Alfelor Sr. Memorial College, Inc. hereby announces that enrollment for the First Semester, School Year 2026–2027 is officially open from June 15 to July 18, 2026. Classes will commence on July 20, 2026.
All concerned students are advised to complete their enrollment within the prescribed schedule and strictly follow the procedures below:
📌 ENROLLMENT PROCEDURE
STEP 1 – Evaluation of Grades (Old Students / Transferees)
Proceed to the Registrar’s Office for grade evaluation and record verification.
STEP 2 – Preliminary Registration Form
Proceed to the Dean’s Office / Department Head / Designated Area
• Claim two (2) copies of ASMC Form 1A (Ver. 3.0, S.Y. 2024)
• Fill out the forms clearly and completely
📍 Designated Area:
• Conference Room, 2nd Floor, ASMC Administration Building
STEP 3 – Assessment of Fees (TOSF)
Proceed to the Admission/Accounting Office
• Secure three (3) copies of your assessment form for:
o Dean’s Office
o Accounting Office
o Registrar’s Office
o Student Copy
STEP 4 – Payment of Fees
Proceed to the Cashier’s Office
• Submit your assessment form
• Pay the required fees
• Secure your Official Receipt (OR)
STEP 5 – Submission of Enrolled Documents
Submit your “OFFICIALLY ENROLLED” stamped matriculation form to:
• Dean’s Office
• Accounting Office
• Registrar’s Office
📎 ADDITIONAL REQUIREMENTS
(For students who have incomplete submissions at the Registrar’s Office)
Please ensure compliance with the following documents:
• Form 137 / Form 138
• School Form 10 (SF10)
• PSA Birth Certificate
• Medical Certificate
• Two (2) pcs. 2x2 ID picture (white background, with collar)
• One (1) long brown envelope
• TOR (Transcript of Records) – for transferees
