This online platform is designed to provide you with a seamless and efficient way to track the status of your requested academic records, including:
* Official Transcript of Records (OTR)
* Reconstituted Diplomas

* Scan the QR Code or visit adms.qsu.edu.ph
* Enter your Student ID Number in the search field.
* Check your status:
> Pending
> Processing
> Ready for Pick-Up

When claiming your documents, please present a valid ID. If you are sending a representative, ensure they have a formal authorization letter, copy of your ID and a copy of ID of the Representative.
