1. Duly Accomplished Admission Form.
2. Original Copy of Learner’s Progress Report Card (School Form 9).
3. Original Copy of Certificate of Good Moral Character.
4. Photocopy of the Senior High School Diploma.
5. Four (4) Copies of 2x2 ID Picture with white background and name tag (Family Name, First Name, and Middle Initial).
6. Photocopy of PSA Birth Certificate.
7. Certificate of Physical/Medical & Dental Examination issued by University Health Services/Infirmary Services.
8. Long brown envelope with name in front (SURNAME, NAME, MIDDLE INITIAL).
