Please be guided by the following guidelines regarding the re-implementation of our campus-wide uniform policy, compliance procedures, and the application for Promissory Forms:

โข Monday & Wednesday: Prescribed School Uniform
โข Tuesday & Thursday: Department T-Shirt
โข Friday, Saturday & Sunday: Wash Days (Appropriate Casual Attire)

โข 1st Offense: Recorded in logbook + Present valid Certificate of Enrollment (COE) when required.
โข 2nd Offense: Recorded in logbook + Present COE + Submit a duly accomplished Promissory Form via online Google Form or printed copy for OSAS review.
โข 3rd Offense: Recorded in logbook + Render community service + Endorsement to Student Discipline Section.
Note: Offense counts are progressive and will not be reset upon submission or approval of a Promissory Form.

โข One-Time Application Rule (2nd & 3rd Year Students): Application for a Promissory Form is allowed only once during the implementation period for the same uniform requirement.
โข First-Year Students Transition: First-year students are given a transition period. Beginning November 16, 2026 (Start of 2nd Semester, A.Y. 2026โ2027), all 1st-year students must strictly wear the prescribed uniform and will be subject to the standard offense-counting system.
โข Validity Period: An approved Promissory Form is valid for one (1) month from approval and serves as temporary authorization only.
โข How to Apply: Apply via the official Google Form (https://forms.gle/bXqSzGytaUdtCfAK9) or obtain a printed form from the designated office.
