2. Access the MENU BAR, then ADMISSION RESULT section. To view your admission results, please complete the details required by the system. This will be used in your official documents.

In line with the extension of the MECQ in the province, all qualified applicants who have already confirmed their slot are advised to SUBMIT ALL THEIR ORIGINAL REQUIREMENTS at the Office of the Campus Registrar until TUESDAY, AUGUST 31, 2021 from 8am - 5pm.

You may submit your credentials via courier service of your choice. For personal submission, please follow all health safety protocols

1. Wear your face mask and face shield
2. Sanitize your hands regularly
3. Observe physical distancing at all times

Submission of COMPLETE original requirements is needed to process your registration/enrollment. Incomplete requirements will not be entertained.

1. Confirm your slot by filling out this google form, https://forms.gle/eJeM7eF9gwYQeJDB8 Failure of confirmation will be automatically removed from the list. Deadline is on August 21, 2021.
2. After securing your slot, prepare the ORIGINAL REQUIREMENTS for submission.
3. STARTING AUGUST 5, 2021, ALL qualified applicants who have CONFIRMED their slots can submit their ORIGINAL REQUIREMENTS for registration.
For Grade 12 Graduates (New First Year Students):
1. ORIGINAL Form 138 or Grade 12 Report Card
2. ORIGINAL Certification of Good Moral Character
For ALS Passers:
1. ORIGINAL Certification of ALS Rating with recommendation for admission to college
2. ORIGINAL Certification of Good Moral Character
For College Transferees/Second Courser:
1. ORIGINAL Transcript of Records or Certificate of Grades signed by the Registrar
2. ORIGINAL Honorable Dismissal or Certificate of Eligibility to Transfer
3. ORIGINAL Certification of Good Moral Character (for transferees only)
4. ORIGINAL NBI Clearance (for students who are 18 years old and above) or Police Clearance (if the student is 17 years old and below) (for transferees only)
2. PRINTED Student Information Sheet with 1x1 ID picture
