𝐒𝐭𝐞𝐩 𝟏: 𝐄𝐧𝐬𝐮𝐫𝐞 𝐄𝐥𝐢𝐠𝐢𝐛𝐢𝐥𝐢𝐭𝐲
* Verify that you are either currently enrolled or have finished all academic requirements in your curriculum.

𝐒𝐭𝐞𝐩 𝟐: 𝐏𝐫𝐞𝐩𝐚𝐫𝐞 𝐭𝐡𝐞 𝐂𝐡𝐞𝐜𝐤𝐥𝐢𝐬𝐭
* Print your checklist (2 copies) back-to-back on a single page.
* Provide accurate information and complete all required fields fully.
* Have it evaluated and verified by your registration adviser.

𝐒𝐭𝐞𝐩 𝟑: 𝐏𝐫𝐞𝐩𝐚𝐫𝐞 𝐭𝐡𝐞 𝐀𝐩𝐩𝐥𝐢𝐜𝐚𝐭𝐢𝐨𝐧 𝐅𝐨𝐫𝐦 𝐚𝐧𝐝 𝐏𝐡𝐨𝐭𝐨𝐜𝐨𝐩𝐲
* Download and print the Application for Graduation form (A4 paper size) from the Google Drive Link.
* Provide accurate information and complete all required fields fully.
* Create a photocopy of your completed application to serve as your receiving copy.

𝐒𝐭𝐞𝐩 𝟒: 𝐒𝐮𝐛𝐦𝐢𝐭 𝐎𝐧𝐥𝐢𝐧𝐞
* Log in using your CvSU email.
* Submit your details via Google Form Link before submitting physical documents.

𝐒𝐭𝐞𝐩 𝟓: 𝐒𝐮𝐛𝐦𝐢𝐭 𝐇𝐚𝐫𝐝 𝐂𝐨𝐩𝐢𝐞𝐬
* Hand in the hard copies of your documents to the CEMDS Registrar's Office.
* Keep your photocopy stamped as your receiving copy for future reference.

Good day, Students.

As part of the pilot implementation of the 𝐎𝐧𝐥𝐢𝐧𝐞 𝐃𝐨𝐜𝐮𝐦𝐞𝐧𝐭 𝐑𝐞𝐪𝐮𝐞𝐬𝐭 𝐒𝐲𝐬𝐭𝐞𝐦, students are reminded to follow the implemented process when requesting documents from the University Registrar’s Office.

Please refer to the steps provided in the post and ensure that you use your official 𝐂𝐯𝐒𝐔 𝐞𝐦𝐚𝐢𝐥 𝐚𝐜𝐜𝐨𝐮𝐧𝐭 when submitting your document request to receive notifications and updates properly.

The documents covered by this process are COR, COG, and COE.

Please see below the schedule for Tuesday’s Graduation Ceremony (September 01, 2026) to ensure proper preparation before the program begins.

Kindly take note of the following reminders:

- Each candidate is allowed one (1) companion only.

- The wristlet is intended for your parent or companion and will be required for entry to the designated venue.

- Please strictly observe the prescribed dress code for the graduation ceremony.

- Candidates are reminded to follow the guidelines on the permitted hair color.

- Please arrive at the designated venue on time and follow the instructions of the assigned faculty and staff members.

Candidates who are unable to personally claim their invitations and wristlets today may ask a classmate or authorized representative to claim them on their behalf. Please note that the distribution will be 𝐮𝐧𝐭𝐢𝐥 𝟏𝟏:𝟎𝟎 𝐚.𝐦. 𝐭𝐨𝐝𝐚𝐲 𝐨𝐧𝐥𝐲. The authorized representative must present a valid authorization letter bearing your e-signature or wet-ink signature, along with a valid ID, upon claiming the items.
