To apply for Tuition Loan/SLB Programs (i.e. Tuition Loan / SLB Promissory Note, Request to Avail of 100% SLB, Tuition Loan / SLB Application), follow the instructions below:

1. Access the OVCSA website at https://uplbosa.org/ and log in to your OSAM Account. It is important to 𝗲𝗻𝘀𝘂𝗿𝗲 𝘆𝗼𝘂𝗿 𝗢𝗦𝗔𝗠 𝗽𝗿𝗼𝗳𝗶𝗹𝗲 𝗵𝗮𝘀 𝗮𝗰𝗰𝘂𝗿𝗮𝘁𝗲 𝗮𝗻𝗱 𝘂𝗽𝗱𝗮𝘁𝗲𝗱 𝗶𝗻𝗳𝗼𝗿𝗺𝗮𝘁𝗶𝗼𝗻, including your 𝗻𝗮𝗺𝗲, 𝘀𝘁𝘂𝗱𝗲𝗻𝘁 𝗻𝘂𝗺𝗯𝗲𝗿, 𝗱𝗲𝗴𝗿𝗲𝗲 𝗽𝗿𝗼𝗴𝗿𝗮𝗺, 𝗮𝗻𝗱 𝗰𝗼𝗹𝗹𝗲𝗴𝗲. If you need assistance in creating an OSAM Account and updating the above details, you need to send an email to RECOMMIT at [email protected]

You are also 𝗿𝗲𝗾𝘂𝗶𝗿𝗲𝗱 𝘁𝗼 𝘂𝗽𝗹𝗼𝗮𝗱 𝗮 𝗿𝗲𝗰𝗲𝗻𝘁 𝗽𝗵𝗼𝘁𝗼 𝗮𝗻𝗱 𝘂𝗽𝗱𝗮𝘁𝗲 𝘆𝗼𝘂𝗿 𝘆𝗲𝗮𝗿 𝗹𝗲𝘃𝗲𝗹, 𝗮𝗻𝗱 𝗽𝗲𝗿𝗺𝗮𝗻𝗲𝗻𝘁 𝗵𝗼𝗺𝗲 𝗮𝗱𝗱𝗿𝗲𝘀𝘀 on your OSAM profile to be able to apply for the Tuition Loan/SLB Programs.

𝙄𝙢𝙥𝙤𝙧𝙩𝙖𝙣𝙩 𝙉𝙤𝙩𝙚: If your OSAM account is not updated or if you submit incorrect requirements, your application will not be prioritized or processed.

2. After logging in, navigate to 𝗦𝘁𝘂𝗱𝗲𝗻𝘁 𝗦𝗲𝗿𝘃𝗶𝗰𝗲𝘀 > 𝗙𝗶𝗻𝗮𝗻𝗰𝗶𝗮𝗹 𝗔𝘀𝘀𝗶𝘀𝘁𝗮𝗻𝗰𝗲 > 𝗧𝘂𝗶𝘁𝗶𝗼𝗻 𝗟𝗼𝗮𝗻/ 𝗦𝗟𝗕 𝗣𝗿𝗼𝗴𝗿𝗮𝗺.

1. Transcript of Records (TOR) or True Copy of Grades (TCG) from student’s home unit (all semesters enrolled)
- Earned at least 30 academic units
- GWA in current program
- If 1st sem 2025-2026 grades are unavailable, enrolled courses should also be reflected on the TCG and a screenshot of grades in CRS/AMIS should be provided

2. Undergraduate Admission Application Form (UPLB Form No. 3)

3. Certificate of Non-Enrollment (if student has a semester with Leave of Absence or Absence Without Leave)

4. Certificate of No Contract

5. Certificate of Good Moral Character

* Transfer applicants should have a good academic standing in last semester enrolled
* Incomplete requirements will not be evaluated
* Submissions through email will not be accepted
* BS Accountancy (not yet offered to all transfer applicants)

We remind applicants that slots are limited. Meeting the minimum requirements does not guarantee acceptance to the program.

REQUIREMENTS:
1. Earned at least 30 academic units

2. True Copy of Grades (TCG)

- Must reflect all courses taken including 1st sem 2025-2026 and GWA.

- If 1st sem 2025-2026 grades are unavailable, enrolled courses should also be reflected on the TCG and a screenshot of grades in AMIS should be provided

3. Letter of intent to shift (approved by current College Secretary/Dean)

4. Accomplished Application for Shifting Form (OUR Form 2021-6)

- Leave Action of the Department blank
