📍 CABHA Building, 2nd Floor, Office of Student Affairs and Services (OSAS)

📌Students who are residents of Lucban may claim their PhilHealth documents directly from the Rural Health Unit (RHU) of Lucban.

📌Please be advised that some PhilHealth documents are still with the PhilHealth Office and have not yet been endorsed to the OSAS. As a result, not all students’ documents are currently available for claiming. Further updates will be provided once the remaining documents are received.
