GUIDELINES FOR SCHOLARSHIP APPLICATION
Eligible applicants are those who:
1. At least an incoming 2nd year college student of Doctor of Veterinary Medicine.
2. Must have an annual family income that does not exceed PHP 600,000.00
3. Must belong to the top 30% of the course batch from the previous academic year
4. Must have a minimum GWA of 2.5, from the previous academic year
5. Must have no failed, dropped, or incomplete grades in any semester.
They must have the following documents on-hand prior to applying:
1. Certified true copy of grades from the previous academic year, showing no failed grade in any subject;
2. Certificate of batch ranking from the previous academic year, showing candidate belongs to the top 30% of college batch;
3. Certificate of good moral standing or a reference letter from the college dean, or department head;
4. Proof of enrollment or Certificate of Registration for the coming semester/term of the academic year; and
5. Parent or family's proof of income (examples: recently filed annual income tax return (ITR), last 2 pay-slips, or Certificate of Indigency)
