APPLICATION REQUIREMENTS FOR FIRST YEAR:
1. Must have completed a Baccalaureate Degree (B.A. or B.S.).
2. Must have taken the NMAT.
3. Must have taken the Entrance Exam prepared by the 4. Admissions Committee and undergone an Interview (Scheduled date will be announced).
4. Must be of good moral character and must not have been convicted of any crime.
SELECTION CRITERIA:
1. National Medical Admission Test (NMAT)
2. Weighted General Average
of 2.5 and above.
(Major basis selection).
3. Interview
OTHER REQUIREMENTS:
1. Completely and accurately filled up application form
2. Three (3) signed 2” x 2” photographs taken within two (2) months of application date
3. NMAT certificate (original and copy)
4. Complete Transcript of Records
5. Birth certificate (NSO)
6. Two (2) certificates of good moral character from the guidance counsellor and Dean or parish priest/pastor
7. Certificate of citizenship for aliens and naturalized Filipinos.
