๐ REQUIREMENTS FOR FILING OF CANDIDACY
1๏ธโฃ Student Application Form โ Template to be provided by the Office of Student Affairs (OSA).
2๏ธโฃ Letter of Intent to Run
3๏ธโฃ Endorsement Letter - Using the Annex A template provided by the OSA, duly prepared by the student, together with an Endorsement Letter signed by three (3) Coaches/Faculty Members.
Letter of Parental Consent โ Using the Annex B template provided by the OSA, expressing the parent/guardian's consent for the student to run for the CSG Election.
4๏ธโฃ Photocopy of RAF
5๏ธโฃ 2ร2 ID Photo
6๏ธโฃ Planned Projects for the STI Community โ Present your proposed initiatives and projects that you intend to pursue for the benefit of the STI community.
