CHE students, please consult with your program adviser on what subjects to include in your ESF.

Important reminders:
1. For MY 2026, please limit your responses to 2-3 subjects only.
2. Check that your student information are correct, complete student ID number, name of your adviser (please input only the last name and first name of your adviser)
3. Editing of responses is until April 6, 2026 (8am) only.
4. Only CHE students are allowed to answer the ESF.

DEADLINE OF DROPPING AND LOA APPLICATION, 2nd sem AY 25-26

Dropping: April 23 (Thursday)
LOA Application: May 7 (Thursday)

For both processes, all steps must be completed on or before the deadline. These include obtaining your instructor's consent, having your adviser “note” your application, having your application endorsed by your dept/college (if applicable), obtaining approval from your dean, and paying the fees indicated on the dropping/LOA form or payment slip.
