The PUP Open University System is now officially accepting applications for the Summer Term.
Requirements for Admission:
1. Letter of Intent addressed to
Assoc. Prof. Ronaldo G. Bulfa
Campus Director and Open University System Coordinator
PUP Lopez, Quezon.
2. Entrance Exam Requirements
a. Printed PUP iApply Form, Reference Number, and
Printed Voucher (No need to pay again, we will just
attach the Original OR of Entrance Exam).
b. Filled-out Recommendation for Admission (If
employed) duly signed by the immediate
supervisor/employer.
3. Admission Record with 2x2 ID picture in formal attire
and in white background
4. Pre-registration Form
5. Honorable Dismissal / Transfer Credential
6. Original Transcript of Record of highest degree
earned from the school last attended (TOR with
remark “Copy for PUP Open University System” shall
be submitted within the semester)
7. Original Copy of Birth Certificate (PSA Authenticated)
8. Original Copy of PSA-issued marriage contract for
married female applicants who opt to use their
husband’s name.
9. Updated Curriculum Vitae/Resume reflecting
research works and publications, if any
10. A two-page write up/concept paper discussing or
describing the research area, idea, or interest that the
applicant would like to investigate in his/her thesis.
The write-up should be on short bond paper and
encoded using Arial font size 12 and double spaced.
11. Two identical copies of recent ID pictures, 2x2 inch
in formal attire and in white background
12. Certificate of Employment or Service Record (if
employed)
13. NBI Clearance/Police Clearance within the last 6
months (if unemployed)
14. Chest X-ray with film and result taken within 3
months
15. Two Long Brown Expanding Envelopes
All the above documents should be submitted in two sets (original and photocopy) in long brown envelope.
