If the recipient is unable to claim personally, the authorized representative must present the following:
1. Two (2) photocopies of the student's School ID, preferably in color and on long bond paper, with three (3) specimen signatures.
2. An ๐๐๐๐ต๐ผ๐ฟ๐ถ๐๐ฎ๐๐ถ๐ผ๐ป ๐๐ฒ๐๐๐ฒ๐ฟ signed by the student/recipient.
3. A ๐๐ฎ๐น๐ถ๐ฑ ๐๐ ๐ผ๐ณ ๐๐ต๐ฒ ๐๐๐๐ต๐ผ๐ฟ๐ถ๐๐ฒ๐ฑ ๐ฅ๐ฒ๐ฝ๐ฟ๐ฒ๐๐ฒ๐ป๐๐ฎ๐๐ถ๐๐ฒ.
4. Two (2) photocopies of the Authorized Representative's valid ID, with three (3) specimen signatures.
Students are advised to prepare all required documents before proceeding to the scheduled release.
