ONLINE ENROLLMENT PROCEDURE FOR GRADUATE STUDENTS (MAED, MPA) ONLY
1. Download the class schedule. Choose the subjects you will take this semester. You will need this information to fill out the enrollment form. Download the schedule here: https://drive.google.com/file/d/1V0RtwjaZylffjeSZPSH50BobBoeCiYyc/view?usp=sharing
2. Download and fill out the enrollment form completely from this link: https://docs.google.com/spreadsheets/d/1XNvBuaf9y3UfriS6V16A6jcOtth_1gLi/edit?usp=sharing&ouid=112857228909862387716&rtpof=true&sd=true

3. Fill out the form on your computer and save the enrollment form. Change the file name to your full name as well as the degree you are seeking. (Eg. Maria Alma Santos MAED.xls)
4. Please prepare the following required documents. Please scan or take a good (hi-resolution, in-focus) photo of the following documents:
New and Transfer Students:
- OTR from previous school (CLEAR SCAN COPY) please submit TOR with valid remarks
- Current photo (square format, min. 150dpi). You can scan a
2x2 photo (white background) or take a digital photo (white
background). Please note that if the photo is not of sufficient quality, it may be rejected.
- Marriage cert for married woman
RETURNING STUDENTS:
- Current photo (square format, min. 150dpi). You can scan a
2x2 photo (white background) or take a digital photo (white
background). Please note that if the photo is not of sufficient quality, it may be rejected.
- Curriculum with previously taken subjects checked off.

5. Email the completed enrollment form to [email protected] and attach the required documents in JPEG format only. Please write your full name and degree sought (MAED, MPA) in the subject line. Eg. MARIA ALMA SANTOS MAED, and all requirements must be the same file name.
6. Kindly wait for a response verifying the subjects and assessments of fees.
NOTE: Don't send multiple emails as it will cause delay just wait.
7. After receiving an email regarding your assessment and verification, please send payment of fees. Payment method and information will be included in the email.
8. After making your payment, please send a clear copy of your payment receipt or confirmation screen to [email protected]. Please write your full name and degree sought (MAED, MPA) in the subject line. Eg. MARIA ALMA SANTOS MAED.
9. Kindly wait for another email that will confirm your successful enrollment.
Please follow the instructions correctly so your application will not be delayed. All submitted requirements should follow the same file-naming format.
