ANNOUNCEMENT!! BS NURSING PASSERS!
We are pleased to inform you that you have successfully passed the BS Nursing Interview and are now qualified to proceed to the Medical Examination, which is one of the admission requirements for the Bachelor of Science in Nursing program.

Please accomplish the attached Medical Examination Form, print a copy, and bring it to your preferred licensed clinic or hospital to undergo the required medical examination. Ensure that all required tests are completed and that the attending physician properly fills out and signs the form.

Kindly submit the completed Medical Examination Form, together with all required medical test results, to the DLSP Clinic before proceeding with your enrollment.

Please note that compliance with the Medical Examination requirement is necessary to continue with the admission process.

Important Reminders:
✅Complete the Physical Fitness Test Readiness Form, print a copy, and bring the accomplished form on your assigned schedule.
