โœ… Filing/Submission of Letter of Intent to Graduate, Application for Graduation and Requisition of Transcript of Records.

(For all graduating students enrolled in the new student portal)

STEPS:
โžค1. Log in to the Student Portal and accurately complete the form in the Application for Graduation tab
โžค 2. Download the duly accomplished Application for Graduation Form (3 copies) and Letter of Intent ( 1 copy) and Print them on A4 Bond paper.
โžค 3. Submit the filled-out forms in hard copy at the designated windows during the scheduled times:
1 copy for the student
1 copy for the College Dean/Associate Dean
1 copy for the Office of the Registrar
โžค 4. Kindly request and accomplish the Request slip for Official Transcript of Records, then submit to the Office of the Registrar.

(For graduating students enrolled in the old student portal)

โžค 1. Download the forms from the links provided, print them on A4 Bond paper, and fill them out correctly.

STEP 1: ACCESS YOUR STUDENT PORTAL
โ€ขUnder the โ€œREQUESTโ€ tab, choose the โ€œCERTIFICATE REQUESTโ€
โ€ขlook for โ€œREQUEST CERTIFICATEโ€ button on the upper right of the screen
โ€ขPlease fill-out the blank fields
โ€ขCertificate Type (choose Certified True Copy)
โ€ขIndicate Purpose
For Scholarship Purpose
For General Purpose
โ€ขNumber of Copies
โ€ขClick โ€œSUBMITโ€ button.

STEP 2: PRINTING
โ€ขPrint the COR/COG that you intend to be certified by the registrar.

STEP 3: PAYMENT
โ€ขPresent your school ID and pay at the cashier.

STEP 4: PROCEED TO THE OFFICE OF THE REGISTRAR
โ€ขPresent the official receipt and the printed COR/COG.
โ€ขClaim the certified document/s.
