Submission Deadline for Admission Requirements for all Qualifiers and Waitlisted of CNSC Entienza Campus!

Attention all applicants! We are excited to inform you about the important dates for submitting your admission requirements. Please make sure to mark your calendars and prepare your documents accordingly.

📅 Important Dates:
June 20-21, 2024 for Qualifiers
8:00 AM - 4:00 pm
July 8, 2024 for Waitlisted
8:00 AM - 4:00 pm

🔗 Requirements for Official Admission and Registration
REQUIREMENTS:
1. ORIGINAL and PHOTOCOPY of High School Report Card (SF 9).
2. ORIGINAL (In Philippine Statistics Authority Security Paper (SECPA) and PHOTOCOPY of your Birth Certificate).
3. ORIGINAL copy of the Certificate of Good Moral Character duly signed by either the School Principal or Guidance Counselor.
4. ORIGINAL copy of Police Clearance.
5. Two (2) Copies of Passport Size Picture with White background and Nametag.
6. (1) White Folder, Long Size & (1) Long Size Brown Envelope.
7. Completely Filled out the Online Medical and Dental Health Inventory Record Form (can be accessed after you select your submission schedule).
8. Completely filled-out Online Individual Inventory Record Form (can be accessed after you select your submission schedule).
9. Original Chest X-Ray Frame and X-ray Result.

Submission Method:
Documents must be submitted in person at the Administration Office of CNSC Entienza Campus
