Please be informed that today is the LAST DAY for the submission of the PhilHealth Konsulta (PKRF) Registration Forms.

All alumni and concerned individuals who have not yet submitted or have not fully accomplished their PKRF forms are strongly advised to visit the LSPU Siniloan Alumni Office as soon as possible. Our office will assist you in completing and verifying your forms to ensure that your documents and records are properly updated and processed.

This form is a necessary part of the PhilHealth Konsulta enrollment process, and your prompt compliance will help ensure the smooth and timely processing of your documents. We kindly encourage everyone concerned to submit the accomplished form as soon as possible to avoid any delays.
