Maaari pong ipagpatuloy ang serbisyo at request sa pamamagitan ng email at SINTA.

Para sa iba pang PUP campuses at academic extension programs, ang ipapatupad na kaayusan sa klase at trabaho ay nakabatay sa mga abiso na ilalabas ng kani-kanilang mga lokal na pamahalaan (LGU). Tandaan na ang abisong ito ay maaaring madagdagan o mabago batay sa karagdagang anunsyo mula sa Presidential Communications Office at iba pang kaugnay na ahensya ng pamahalaan.

Dear Incoming PUP Taguig First-Year Students,

Greetings!

Please be reminded on the submission of your medical requirements through the CareSync Online Clinic Management System.

To avoid delays in your medical clearance and enrollment process, please ensure that all required documents are uploaded and that you bring the necessary physical documents on your scheduled medical assessment.

How to Access the CareSync Clinic System

1. Visit the PUP One Portal: https://one-portal.isaxbsit2027.com/landing

2. Log in using your PUP account credentials.

3. Open the Clinic System.

4. Complete your Health Information Form.

5. Upload all required documents.

6. Download and print your accomplished Health Information Form.

Required Documents to Upload

Please upload clear and readable copies of the following:

- 2ร—2 ID Picture
- Medical Certificate (must indicate "Fit for Enrollment," "Fit for School," or "Fit for College")
- Chest X-ray Result (Radiology Report only)
- Declaration of Medical Information and Data Subject Consent Form (signed)

ยซNote: Do not upload the Chest X-ray film. Only the official Chest X-ray Result (Radiology Report) should be uploaded.ยป

How to Upload the Declaration of Medical Information and Data Subject Consent Form

After signing the Declaration Form:

1. Log in to the PUP One Portal.
2. Open the Clinic System.
3. Go to My Account.
4. Select Health Record.
5. Click the Upload Declaration Form button.
6. Select your signed Declaration of Medical Information and Data Subject Consent Form.
7. Click Upload and wait for the confirmation that your file has been successfully uploaded.

Documents to Bring During Your Scheduled Medical Clearance

Please bring the following original documents:
- Current Medical Certificate
- Chest X-ray Result (Radiology Report)
- Chest X-ray Film
- Printed Health Information Form downloaded from the CareSync Portal
- Health Information Form attached to your Student Admission Record (SAR)
- Student Admission Record (SAR)
- Route and Approval Slip (RAS), signed up to Step 2
- Declaration of Medical Information and Data Subject Consent Form

Please also prepare one (1) photocopy of the following:

- Medical Certificate
- Chest X-ray Result
- Student Admission Record (SAR)
- Route and Approval Slip (RAS)

There is no need to photocopy the following:

- Health Information Form downloaded from the CareSync Portal

- Health Information Form attached to the SAR
- Declaration of Medical Information and Data Subject Consent Form
- Chest X-ray Film

Important Reminders

- Upload all required documents before your scheduled medical assessment.
- Applicants 18 years old and above may sign the Declaration Form themselves.
- Applicants below 18 years old must have the Declaration Form signed by their parent or legal guardian.
- Failure to complete the online submission or bring the required documents may result in delays in your medical clearance and enrollment process.

Bring the following documents in long/A4-sized bond paper:
โœ… New Student Admission and Enrollment Procedure (1 copy)
โœ… Signed Student Admission Record (SAR)-Form 1 (1 copy)
โœ… Route and Approval Slip (1 copy)
โœ… Grade Verification Slip (1 copy)
โœ… Admission Requirements (Original and 3 photocopies)

โš ๏ธ Important Reminders:

๐Ÿš‘ Medical requirements shall be submitted on your program's scheduled medical clearance processing.
๐Ÿšซ Camping on campus is strictly discouraged. Arrive 1โ€“2 hours before your scheduled interview time.
๐Ÿš— No parking space for applicants โ€” applicants may be dropped off at the entrance gate.
๐Ÿ‘• Dress code reminder: Maintain decency and proper grooming. Prohibited clothing includes tokong shorts, short pants, sando, leggings, spaghetti strap dresses, mini skirts, and sleeveless tops.
