If you are unable to claim your cash incentive personally, please do the following:
1. Prepare and send to your authorized representative:
-Authorization Letter with signature
-Any valid government-issued ID with 3 specimen signatures and send it to your family/authorized rep
2. Upon claiming the cash incentive, your authorized representative must:
-Show proof of your conversation (chat/message) confirming that they are the person you authorized to claim the incentive.
-Submit 1 photocopy of any valid government-issued ID of the authorized representative with 3 signatures
3.Authorization letter mentioned in number 1
