2nd Semester AY 2025-2026 Enrollment Schedule will be on Dec. 22- Dec. 23, Dec. 26.

*To all first year students, please proceed directly to your respective department, get your pre-enrollment form, and submit your pre-enrollment form at the Registrar's Office.
*To all higher years (2nd year-4th year), please present your grades to your respective department, get your pre-enrollment form and then submit your pre-enrollment form at the Registrar's Office.

BSA 3rd Year: July 21-22, 2025
BSA 4th Year: July 29, 2025
BEEd 2nd Year and 3rd Year: July 23-24, 2025

For the Incoming First Year Students, enrollment will start on July 1, 2025.

*The students need to pass the Entrance Examination and Interview first before they can proceed with the enrollment.

PROCESS FLOW:

1. Please get your ADMISSION CLEARANCE at the Guidance Office.
2. Go to the School Clinic to submit the following requirements:
-Medical Results (CBC, X-Ray, Urinalysis, Drug Test, Blood Typing)
-2 pcs. Long Brown Envelope
-1 pc. 2x2 pic
-Vaccine Card
3. After submitting all the Medical Results at the School Clinic, proceed to the Registrar's Office for the submission of requirements:

For K-12 Graduates:
1. 1 pc. 2x2 pic
2. 2 Long White Folder
3. 1 Original copy and 3 photocopies of Report Card
4. 1 Original copy and 3 photocopies of Good moral
5. 1 Original copy of NCAE (if available)
6. 3 photocopies of PSA birth certificate
7. 2 photocopies of HS Diploma

Transferees:
1. 1 pc. 2x2 pic
2. 1 Long White Folder
3. 1 Original copy and 3 photocopies of TOR (Transcript of Records)
4. 1 Original copy and 3 photocopies of Good moral
5. 1 Original copy of Honorable Dismissal
6. 4 photocopies of PSA birth certificate
