Dog Bite – Rabies Post-Exposure Prophylaxis (PEP)

1. Immediate Wound Care (MOST IMPORTANT)
Wash thoroughly with soap + running water for at least 15 minutes
Apply antiseptic (e.g., povidone-iodine / chlorhexidine)
Avoid tight suturing (delay if possible)
This step alone can reduce viral load significantly

2. Tetanus Prophylaxis
Tetanus toxoid (TT): 0.5 mL IM
Add tetanus immunoglobulin if indicated (dirty wound + incomplete immunization)

3. Rabies Vaccine (Active Immunization)
IM (Essen regimen):
Days: 0, 3, 7, 14, 28
Dose: 0.5 mL IM in deltoid (anterolateral thigh in children)
Alternative (ID regimen, where practiced):
0.1 mL ID at 2 sites on days 0, 3, 7, and 28

4. Rabies Immunoglobulin (RIG) – Passive Immunization
👉 Indication:
ONLY for Category III exposure
Transdermal bites/scratches
Licks on broken skin
Bites on face/head/neck
Multiple bites

5. 💉 Dose of Rabies Immunoglobulin (IMPORTANT)
There are two types:
🧪 Human RIG (HRIG)
Dose: 20 IU/kg body weight
🧪 Equine RIG (ERIG)
Dose: 40 IU/kg body weight

6. Administration Technique
Infiltrate as much of the calculated dose as possible into and around the wound
Any remaining volume → inject IM at a site distant from vaccine site
Do NOT mix RIG and vaccine in the same syringe or site

7. Timing
Give RIG as early as possible (preferably Day 0)
Can be given up to 7 days after starting vaccine
After day 7 → NOT recommended (antibodies already forming)
