ปัญหาที่น่าปวดหัวปัญหาหนึ่งสำหรับคนที่ดูแลโรคไตและโรคเบาหวานคือ ผู้ป่วยเบาหวานที่มีปัญหาทางไตนั้น โรคไตอาจจะเกิดจากตัวเบาหวานเอง (diabetic kidney disease) หรือเกิดจากโรคไตอื่น ๆ ก็ได้

เกณฑ์คร่าว ๆ ของลักษณะ atypical diabetic nephropathy ได้แก่

- no DR หรือ DM onset
