มะเร็งท่อน้ำดี รักษาหายมั้ย?

เป็นคำถามที่ถูกถามบ่อยๆ จะขอเล่าผลการผ่าตัดรักษามะเร็งท่อน้ำดีจากรายงานที่ตีพิมพ์จากภาควิชาศัลยศาสตร์ มหาวิทยาลัยขอนแก่นตั้งแต่อดีตถึงปัจจุบันเพื่อให้เห็นภาพการเปลี่ยนแปลง
ปี 1997 ตีพิมพ์รายงานผลการรักษาคนไข้มะเร็งท่อน้ำดี 125 คนที่มีอาการตัวเหลืองตาเหลืองมารับการผ่าตัดแทบทั้งหมดอยู่ในระยะลุกลามแล้วหลังผ่าตัด median survival time 90 วัน, 1-year survival rate 15% เท่านั้นเรียกว่าสมัยนั้นคนเป็นมะเร็งท่อน้ำดีที่ตัวเหลืองแทบไม่มีโอกาสหายเลย https://pubmed.ncbi.nlm.nih.gov/9656356/

ปี 2008 ตีพิมพ์รายงานผลการผ่าตัด major hepatectomy ใน perihilar ยุคแรกๆที่ยังไม่มีการทำการระบายน้ำดีก่อนผ่าตัดคนไข้ 30 รายมีค่าbilirubin ก่อนผ่าตัดเฉลี่ย 19 mg/dl หลังผ่าตัดมี1-year survival rateเพิ่มขึ้นเป็น 12% และมี 4-year survival rate 6.7%

ปี 2015 ตีพิมพ์รายงานผลการผ่าตัด major hepatectomy ใน perihilar cholangiocarcinoma 153 รายในยุคที่มีการระบายน้ำดีก่อนผ่าตัดแล้ว ผลการผ่าตัดได้ R0 resection 43% และมีโอกาสหาย 5-year survival rate 20.6% โดยมี post-operative mortality 2% งานวิจัยนี้รายงานปัจจัยสำคัญต่อการรอดชีวิตคนไข้คือ Lymph node metastasis และ Positive margin (R1) https://www.ncbi.nlm.nih.gov/pubmed/26691730

ในปีเดียวกันมีงานวิจัยที่เขียนสรุปไล่เรียงพัฒนาการของผลการรักษามะเร็งท่อน้ำดีไว้ตามนี้ https://www.ncbi.nlm.nih.gov/pubmed/24464976

ในปีนี้ 2024 เราตีพิมพ์งานวิจัยล่าสุดรายงานผลการผ่าตัดรักษามะเร็งท่อน้ำดีจากโรงพยาบาลศรีนครินทร์ 1,091 รายตั้งแต่ปี 2002-2021 แบ่งเป็น 2 ช่วงก่อนและหลังปี 2013 ในยุคหลังมี 5-year survival rate หลังผ่าตัดดีขึ้นมากเป็น 40.2% เทียบกับ 12.6% ในยุคก่อน สาเหตุคือการผ่าตัดเราดีขึ้นเห็นได้จากเราได้ R0 ถึง 60% ในขณะที่ 90-day mortality แค่ 5.5% และที่สำคัญซึ่งน่าจะมีผลจากการทำงานอย่างหนักของ CASCAP project ตั้งแต่ปี 2013 ทำให้ยุคหลังมีคนไข้ที่มาจากการ screening ถึง 15% และมี stage 0,1 ถึง 29% รายละเอียดสามารถติดตามอ่านในงานวิจัยตัวเต็มได้ตามนี้ครับ https://doi.org/10.1186/s12957-024-03573-5

ถึงจุดนี้น่าจะสามารถพูดได้แล้วว่าด้วยเทคโนโลยีการผ่าตัดรักษาในปัจจุบันทำให้โรคมะเร็งท่อน้ำดีกลายเป็นโรคที่รักษาหายและมีความปลอดภัยในการรักษาสูง ต้องขอบคุณอาจารย์แพทย์ผู้บุกเบิกและพัฒนามาตั้งแต่เริ่มและทีมงานทุกคนที่ช่วยกันสานต่อมาจนวันนี้ เราตั้งใจจะพัฒนาต่อไปเพื่อประโยชน์ต่อผู้ป่วยและประชาชน
