Make sure you fill out your application — called the DS-11 form — before your appointment for the speediest experience! Blank forms are available in 108 Lippincott Hall, or you can fill out and print off a form — single sided, please! — from travel.state.gov.
Only U.S. citizens can get U.S. passports, so we need to see proof of citizenship to issue you one. Acceptable documents include a certified copy of your birth certificate — not a photocopy! — a previous U.S. passport, or a certificate of naturalization. See a complete list at travel.state.gov.
The agent helping you with your passport application will need proof that you are who you say you are, so don’t forget to bring a government-issued photo ID like a valid driver’s license. (No temporary paper licenses allowed!) If your driver’s license is not from the state of Kansas, make sure to bring your KU ID too.
Everybody wants their cut, so don’t forget to bring payment to your appointment! Fees for the U.S. Department of State are listed on travel.state.gov, as well as on KU’s Passport Center website (see link in bio). Your payment to the Department of State MUST be a check (personal or cashier’s) or a money order, and the amount MUST be exact. You’ll also owe KU a $35 fee — or $50 if we are snapping your pic — which will be paid separately. We take check, card, cash, or money order.
