Cervical cancer remains a major public‑health challenge across the Asia‑Oceania region, particularly within low‑ and middle‑income countries. While cervical cancer is highly preventable, inequities in access to HPV vaccination and screening services continue to persist.

【最新消息】港大醫學院揭示全球高齡女性不孕症負擔持續增加 倡及早生育規劃及評估生育能力

港大醫學院臨床醫學學院婦產科學系研究團隊揭示,全球35至49歲受不孕症影響的女性人數,預計將由2023年的5,360萬人增加至2036年的7,964萬人,增幅約五成。隨着社會經濟轉型與延遲生育日趨普遍,不孕症的負擔正逐步轉向較發達經濟體,當中以東亞地區的負擔最高。研究結果為香港及全球評估不孕症負擔、制定生殖健康政策及優化資源配置提供重要科學依據,並突顯及早介入的重要性,包括及時進行生育力評估、支持生殖系統疾病的早期治療等。

東亞地區不孕症負擔居全球首位

港大醫學院研究團隊利用《全球疾病負擔研究》(Global Burden of Disease,GBD)2023年的數據,分析1990年至2023年間全球204個國家和地區35至49歲女性的不孕症情況,比較各地不孕症患病率、對健康造成的影響,並評估各地不孕症情況的改善空間,同時推算未來的趨勢變化。

研究發現,2023年全球約有5,360萬名35至49歲女性受不孕症影響,即平均每十萬名同年齡女性中約有6,900人面對不孕問題。過去33年間,全球高齡女性不孕症個案持續上升,雖然高社會人口指數(SDI)地區與低SDI地區的不孕症負擔差距較1990年收窄23.1%,但隨着社會經濟轉型和延遲生育日趨普遍,不孕症負擔正逐步由發展水平較低的地區轉向較發達經濟體。在全球21個按GBD分類的地區中,東亞地區的高齡女性不孕症負擔最高,2023年的年齡標準化患病率達每十萬人12,030宗,遠高於全球平均水平。

香港不孕防治須由治療走向預防

香港屬於東亞及高SDI地區,而遲婚、遲育情況也越趨普遍,平均生育年齡位居全球較高水平,意味著未來對生育力評估、輔導及輔助生殖服務的需求或持續增加。

研究指出,香港過往的生殖健康規劃較多集中於確診不孕症後的治療,對生育教育、風險識別、及時評估及預防措施等早期工作的重視相對不足。研究團隊認為,香港這類高度發展地區不應只着眼於治療,而應採取更主動、更具前瞻性的策略,包括加強生育力教育、推動及早生育規劃,以及促進生殖健康問題的早期識別和介入,在女性不同人生階段提供適切支援。研究同時為香港提供全球基準參考,有助評估本地生殖健康負擔、服務覆蓋情況及識別政策改善空間。

港大醫學院臨床醫學學院婦產科學系副教授李凌君教授表示:「高齡女性不孕症並非單純的臨床問題,更關乎家庭福祉、生育自主、勞動人口規劃及長遠社會發展。由於延遲生育及家庭結構轉變,不孕症的挑戰在香港日益受到關注。我們的研究提供國際基準,為香港的生殖健康資源分配提供參考,助政策制定者評估現有服務是否具備足夠的可及性、及時性及公平性,尤其是部分女性可能因經濟成本、認知不足,或工作與家庭責任衝突而延遲求診。」

李凌君教授續指,研究結果支持建立以生命歷程為基礎的生殖健康策略,包括提升生育力認知、預防及早期治療生殖系統疾病、適時轉介機制、公平獲取生育服務,以及制定更完善的政策支援女性和家庭作出適切的生育規劃和選擇。

籲及早介入 加強生育教育

團隊進一步推算,若現有趨勢持續,至2036年,全球35至49歲女性不孕症個案數目增加約一半至約7,964萬宗,而該疾病對健康和生活質素造成的影響亦會同步上升。研究結果顯示,高齡女性不孕症將成為未來全球重要的公共衞生挑戰,突顯及早評估生育能力、及早規劃生育,以及加強生殖健康服務的重要性。

李凌君教授表示:「這項研究是根據歷史數據及現有趨勢作出推算,而非對未來情況的精確預測。儘管如此,研究結果清楚顯示,高齡女性不孕症問題正持續加劇,值得社會高度關注。我們有必要採取更早期及更具針對性的公共衞生措施,以應對延遲生育為生殖健康所帶來的影響。」

We are delighted to share the latest research findings from Professor Queenie L.J. Li’s team, published in the prestigious journal 𝑻𝒉𝒆 𝑳𝒂𝒏𝒄𝒆𝒕 𝑶𝒃𝒔𝒕𝒆𝒕𝒓𝒊𝒄𝒔, 𝑮𝒚𝒏𝒂𝒆𝒄𝒐𝒍𝒐𝒈𝒚, & 𝑾𝒐𝒎𝒆𝒏'𝒔 𝑯𝒆𝒂𝒍𝒕𝒉.

Using data from the Global Burden of Disease (GBD) 2023 study, the research team examined the impact of infertility among women aged 35–49 in 204 countries and territories over a period of 33 years (1990–2023).

𝐊𝐞𝐲 𝐅𝐢𝐧𝐝𝐢𝐧𝐠𝐬:

• In 2023, an estimated 53.6 million women aged 35–49 worldwide were affected by infertility, with the disease burden steadily increasing over the past 30 years.

• Although the gap in infertility rates between low- and high-income regions has narrowed by 23.1% since 1990, the burden is increasingly shifting towards more developed economies.

• If current trends continue, infertility cases among older women are projected to rise by approximately 48.6% by 2036, highlighting the urgent need for proactive public health planning.

Given that the average age of childbearing in Hong Kong is among the highest globally, the research team emphasizes that local reproductive health strategies have traditionally focused on treating infertility, leaving room for improvement in fertility education, risk identification, and early assessment.

This study provides valuable international benchmarks to guide reproductive health policy and resource allocation in Hong Kong, supporting the development of a more proactive, life-course approach to women's reproductive health.

研究團隊運用《全球疾病負擔研究》(Global Burden of Disease, GBD)2023 年數據,對全球 204 個國家和地區 35 至 49 歲女性的不育症負擔,進行了長達 33 年(1990 至2023 年)的全面分析。

研究要點:

• 2023 年,全球估計有 5,360 萬名 35 至 49 歲女性受不育症困擾,過去 30 年間相關疾病負擔持續上升。
